The issue you encountered, it's a FTP one, not WordPress. Caused of some internet upload fluctuation, or something. The ftp client (filezilla), but happens to other, too, tried to upload a specific one, got in timeout, then, skipped, on reloading queue, as the file name was in place.
Happened to me too, when uploading mass load of files...
Files with 0 size should not exist... except if they are for some logging issues. Do you need logs?:D

I had a variation of the same problem too after rewriting my .htaccess file on a 301 on version 2.2. The hidden formatting buttons magically appeared - and killed the posting window. Meaning it was no longer "live" - I could get a cursor in the title field, everywhere else on the page, but not to post. So I took down the htaccess changes and the problem corrected itself.

I've had this happen before when I, too, was lazy and didn't delete files before uploading. It also happens when there is a small glitch in the Internet connection. Since I'm on a satellite connection right now, I upload slowly and check everything for this very reason.

Good for you on following through on this!
